xforms:upload problems

Alex/Erik,

I'm trying to test out the new xforms:upload with replace="instance" and
I ran into a problem ...... It seems I can no longer wrap the upload
with a group and conditionally display/hide the upload.

See the attached view.

I found a couple of problems and fixed them. Let us know when you have a
chance to try the latest CVS / nightly.
