w9 PDF sample with form controls

Hello,

When trying to open the Attached PDF for the W9 example. The form PDF fields do not have the form control names as explained in the PDF Template instructions. I have been searching for the .pdf file that has this to help me understand this example better but have not been able to find it.

Can you provide?

I was able to figure it out by editing the PDF. The input field names are actually defined with the form controls. I was under the impression that form was to be filled out using the control names as form field inputs.

Glad you figured it out. The control names, as defined in Form Builder, and the field names in the PDF must match. There is some documentation here in case you haven't seen it:

It was quite clear - I was just looking at the example document picture and interpreting the instruction that literally says to "name the fields" as filling out the fields with the control names. It all makes sense now :)
