Never mind - been having late nights and my problem was actually a typo.  I had the instance for template referenced wrong.  Instance id was service_template and in my insert I had service-template.  

Form Runner and Form Builder are working great.  Actually love starting the forms in form builder and adding other functionality outside and copying back in using Edit Source.
