Wayne,

Somehow, I am getting a message "This file has been deleted" when
following the links to the screenshots you mentioned. Have you added a
language other than English for your form? If you did, you might want
to switch to that language (or languages):

And check that you have a title for every section. You'll let us know
if this helps,
