bug: referencing a nested bind from a repeat causes duplicate items

If I reference a bind from an xforms:repeat, and this bind is nested inside another bind in the model, the repeat items are output twice.

Attached is an example showing the problem. Workaround is to not use nested binds.

However the XForms specification is not clear as to what referencing a
nested bind (by id) means. IIRC the XForms Working Group is aware of the
issue but I am not sure if it has been resolved yet.
