XHTML serializer generates two default namespace declarations

  Hi

  I use ops-3.0.1.200602032326.  In epilogue-servlet.xpl, I
commented out the section outputing plain HTML and
uncommented the section outputing XHTML:

    <!-- BEGIN ASSUME SOME XHTML CLIENTS -->

  When I try to access the OPS documentation in local, for
example http://localhost:8080/ops/doc/processors-xmldb, I
got the following error:

    XML Parsing Error: duplicate attribute
    Location: http://localhost:8080/ops/doc/processors-xmldb
    Line Number 213, Column 65:       <p
xmlns:p="http://www.orbeon.com/oxf/pipeline" xmlns=""
xmlns="http://www.w3.org/1999/xhtml">

  I checked the generated XHTML, and this problem is really
there.  Something seems to be wrong with the serializer.
