Not sure if it is documented anywhere, but Orbeon will get the dtd
from the url specified. It must do this to identify entities.
The error you are seeing may be that the server cannot connect to www.w3.org .
You can get the .dtd and .ent files from w3.org using GET or your
browser and put them onto the local filesystem. Then change the url
in the doctype declaration, e.g. "/xhtml1-strict.dtd"
