I am looking at the second input, which is the one that is more likely
to work first:

<xforms:input ref="test/widgetList/widget[@code='foo']/name">
  <xforms:label>Child of widget named</xforms:label>
</xforms:input>

So you are saying that the input control does not show? What is the
current element? What if you use an absolute expression
(/root/test/...)?
