You want to use an xforms:group instead of xforms:switch/xforms:case
here. See your example modified attached. Note that the controls
should be bound to some other node. In the current code they are bound
to <control> which specifies what type of control is to be used. You
may want to a <value> or use a separate instance to collect the actual
answers.
