I tried with the latest code base and the PDF shows properly. Which Orbeon Forms version are you on right now?

I tried with your form and PDF. There are errors (calling missing service) when the form loads, but the PDF shows ok.

I would think that MySQL doesn't change anything.

With the older version of Orbeon Forms, I am getting the error. I tracked it down to issue #1949 fixed in Orbeon Forms 4.7.1:

Using this version of the file, things work:

I am not sure of the specific scenario triggering the error. Clearly the error didn't happen all the time or the feature would have been entirely broken. There were a few issue which have been fixed in PDF template production since 4.6.
