How to deploy form created by form builder to separate web application

I have created form using form builder and deployed in separate web
application as suggested by doc.

When I execute the same URL configured in page-workflow.xml, display error
on browsers.

Is there anything that needs to be configured before deploying the form.

Please suggest !
