Form Builder-Form Settings: disable Form name and App name fields

Hi Erik and Alessandro,

Would you know if there is a way to disable the input fields* Application
name* and *Form Name* of the Form Settings screen inside Form Builder?  We
would like to make them *read only* as we provide their values automatically
on behalf of the end-user.

Hi Disanka,

Yes, since 4.10, you can pass the app/form name using the `fr-app` and
`fr-form` request parameters, and if both are provided, users won't see that
initial dialog. More on this at:
